Your plan pays 100% of covered hospital expenses for up to 120 days in semi-private hospital accommodations.  Other inpatient and outpatient services are also covered in full.  Inpatient psychiatric care is paid at 80% for up to 60 days.

After a $100 individual calendar year deductible ($300 family maximum), major medical pays 80% of covered charges.  The major medical lifetime maximum is $1,000,000.  Outpatient psychiatric care is paid at 50% to a yearly maximum of $1,000.

Your dental plan pays 100% of the eligible expenses for diagnostic and preventative services such as oral exams and prophylaxis.  For restorative services such as Endodontics and Periodontics, the plan pays 80% for major services such as fixed bridgework and dentures, the plan pays 50% of the fee.  The maximum dental benefit is $1,000 per year.
