The Supplemental Life Insurance program can provide your employees with important financial security by protecting families and the assets accumulated throughout their working lives in the following way:
Additional permanent life insurance at low group
    term cost.
Total ownership.  If your employees leave the
    company they can take their coverage
    with them at the same reduced cost.
The ability for your employees to have the
    option to buy coverage for their spouse,
    dependent children, and grandchildren.
Convenience of payroll deduction.
No medical questions asked when coverage is
    qualified issue during the open enrollment period.
With all of the protection this program can provide at low group cost,
    everyone can have this great benefit.

Coverage Available:
For the Employee
Apply for Portable Life coverage from $25,000 to $100,000 in $25,000
    increments.
Proof of good health must be provided if the employees apply for coverage
    of more than two times their annual salary to a maximum of $1000,000 if
    the employees are less than age 60, two times your annual salary to a
    maximum of $50,000 if the employee is age 60-64, or two times your salary
    to a maximum of $25,000 if the employee is age 65 or over; answer "yes"
    to the health question on the enrollment form; apply for this coverage
    outside of an employer-defined enrollment period; or if participation
    requirements are not met.
Accidental Death and Dismemberment coverage is available in amounts
    equal to Portable Life coverage.
Coverage ends at age 70 for terminated employees who have continued
    their coverage.
For employees actively at work, benefit amounts reduce to 65% of original
    coverage at age 70 and 50% of original coverage at age 75.

For the Employees Spouse
If the employees spouse is under age 65, he/she
    may apply for Portable Life coverage from $20,000
    to $50,000 in $10,000 increments.
Spouse may apply for Portable Life coverage even
    if employee does not.
Separate spouse enrollment form must be
    completed and proof of good health must be
    provided.
Accidental Death and Dismemberment coverage is
    available in amounts equal to Portable Life
    coverage.
Spouse coverage terminates at age 70.

For the Employees Children
Children's Portable Life coverage available in
    amounts of $5,000 or $10,000 if the employee or spouse are covered.
The employee or spouse may apply for children's coverage, but not both.
Benefit limited to 10 percent of elected amount for children age 14 days to
    6 months.
