Employees Select Their Benefit
Amount
Employees select the maximum weekly benefit amount for their plan, from
$100 to $1000 per week to be paid if absent from work due to an illness
or accident.
Benefit Period
In the event of a covered disability, employees
select the benefit period best suited for them - options include 90 days.
6 months, 1 year, two years and more.
Elimination Period
The elimination period can be as little as
7 days to 1 year - the employee decides which option best suits their needs.
Direct Payment of Benefits
Benefits for covered disabilities are paid
directly to the employee to spend as they want.
Premium Waiver
From the first day of the month after the
employee is totally disabled for at least 90 days and are receiving benefits
under this plan, no premiums are due from the employee until benefits end.
Definition of Total Disability
The employee must be unable to perform all
of the duties of their occupation, be under the regular care and attendance
of a legally qualified physician, and not be receiving earnings for performing
any work or service. Loss of license to perform an occupation does
not in itself constitute total disability.
Partial Disability Benefits
This plan pays benefits for partial disability
equal to 50% of the employees regular weekly benefit up to 13 weeks after
a covered total disability. Partial disability means, as a result
of the injury or illness which caused the employees covered total disability,
they are able to perform: a) one or more, but not all, of the main
duties of their occupation, on a full-time or part-time basis, or b) any
of the main duties of their or any other occupation on a part-time basis.
In order to receive benefits for partial disability
the employee must be partially disabled within 30 days of the end of a
total disability for which benefits are payable; earn less than 80% of
their insured weekly earnings, remain under continuing care of a physician,
and submit proof of the continuing partial disability. Partial disability
benefits will be paid until the date the employee ceases to be partially
disabled; payment of 11 weeks of partial disability; or the date the maximum
benefit period ends, whichever occurs first.
Recurrent Disability
When the same or related disabling condition
for which the employee previously received benefits reoccurs within 14
days, the condition will be considered part of the original disability,
and benefits will resume immediately.
Pre-Existing Conditions
Benefits are paid for pre-existing conditions (as defined) equal to 50%
of the employee weekly benefit amount for a maximum benefit period of 5
weeks. Benefits for a pre-existing condition (one that required medical
treatment or advice during the 12 month period prior to the effective date
of the employee coverage under this plan) are available.
Benefits for a pre-existing condition are payable when the disability
begins during the first 24 months of coverage. The pre-existing condition
benefit will be 50% of the weekly benefit amount. The maximum benefit
period for a pre-existing condition is five weeks. When the disability
begins after the first 24 months of coverage, benefits are paid the same
as a sickness.
